Pharmacotherapy has been shown to change the natural history of HFrEF. HFpEF however, has been identified as major public health issue and to date, the etiology, diagnosis, characterization and treatment has remained challenging. Goals of HF therapy include improving survival and reducing morbidity such as hospitalizations and symptoms, while improving functional capacity and quality of life.
Figure 4 outlines a therapeutic approach to patients with HFrEF that is considered optimal medical therapy and defined as GDMT throughout this section.

Previous research found evidence for such a magma ocean on the early moon in the composition of the lunar crust, which is best explained as the floating residue of molten rock that crystallized under lunar conditions, Maurice explained.
In the new study, the researchers developed a model for the solidification of the ancient lunar magma ocean that “considers the influence of many processes occurring back then, some of them disregarded until now,” Maurice said. One such process was mantle convection — the way eddies can form and churn in solid mantle, “which, on the Earth, causes volcanism,” he said.
Isotopic data for the Hf- W system with a Hf half-life of approximately 9 Myr can be interpreted in such a way that the core was formed 34 Myr after the origin of the solar system assuming complete core-mantle equilibrium. Similar estimates on the basis of the U-Pb isotopic system suggest a significantly longer mean time of core formation of approximately Myr. The discrepancy between the U-Pb and Hf-W systems can be resolved assuming prolonged differentiation of prototerrestrial material into silicate and metallic phases, which occurred not simultaneously and uniformly in different parts of the mantle.
This resulted in the isotopic heterogeneity of the mantle, and its subsequent isotopic homogenization occurred slowly. Under such conditions, the mean isotopic compositions of W and Pb in the mantle do not correspond to the mean time of the separation of silicate and metallic phases. This is related to the fact that the exponential function of radioactive decay is strongly nonlinear at high values of the argument, and its mean value does not correspond to the mean value of the function.

However, evidence from isotopes other than radiocarbon shows that dinosaur fossils are indeed millions of years old. Fossil bone incorporates new radiocarbon by means of recrystallization and, in some cases, bacterial activity and uranium decay.
Because of this, bone mineral — fossil or otherwise — is a material that cannot yield an accurate radiocarbon date except under extraordinary circumstances. Science educators need to be aware of the details of these phenomena, to be able to advise students whose acceptance of biological evolution has been challenged by young-Earth creationist arguments that are based on radiocarbon in dinosaur fossils.
The recent discovery of radiocarbon in dinosaur fossils has the potential to generate much puzzlement, because radiocarbon has a half-life too short for measurable amounts of original radiocarbon to remain in fossils that are millions of years old. Many of the other dinosaur-based anti-evolution arguments from YEC authors are less worrisome, because they are plainly absurd e.
That is because students and science educators often lack knowledge of the finer details of radiocarbon dating and the fossilization process that show how radiocarbon in dinosaur bones is consistent with an age of millions of years. Appropriate responses to such YEC arguments are therefore not always at hand.
